Step 1
Receiving Reports
Reports are received through the following channels:

Step 2
Assessment
Each report is reviewed, evidence is verified, and we seek corroboration where possible. Multiple sources demonstrate stronger evidence.​
If the report is weak we may hold it unpublished until more information is available. Alternatively we may publish with a caveat.

Step 3
Categorisation
Each report is classified into broad categories to make searching more effective.
Our categories are not indicative of the severity of the incident or activity, but merely to highlight and group certain areas of focus.
We will continue to monitor and review our classification system to ensure it maintains relevance.
Step 4
Risk Level
We have a simple tricolour system:

Grey - Low Risk

Reports indicating concerning behaviour or emerging patterns, but no regulatory findings or actions have been taken at this stage.​

Orange - Medium Risk
-
Reports of more serious or repeated issues where regulatory oversight, penalties, or compliance directions have been issued.

Red - High Risk
-
Reports of severe or systemic misconduct supported by major regulatory action, such as sanctions, suspensions, or the removal of registration.
